Please complete the following form to the best of your ability. Although it is in your favor to be complete, many exceptional candidates fill out only a fraction of the form so don't feel pressured. When you submit the form it will be automatically e-mailed to a MHAV staff member for review.

Scholarships and funding for these CELT academies are provided by the Department of Behavioral Health and Developmental Services (BHDS). Please note to be eligible for this scholarship, applicants must:

·  Submit a complete application by the application deadline
·  Be a resident of Virginia (proof of residency is required)
·  Be at least 90 days post crisis
·  Be well enough on their recovery spectrum to participate
   in experiential learning
·  Submit one letter of recommendation

Acceptance is not guaranteed and is based on availability. Incomplete applications will not be considered. Scholarships are highly competitive.
